Dr. Kolev's monographic work is relevant for Bulgaria due to the constant negative indicators due to both the low birth rate, especially among the Bulgarian ethnic group, the emigration processes and the high percentage of induced or spontaneous abortions and premature births. Premature births are one of the main causes of high morbidity among newborns.
The work is written on 110 typewritten pages as the cited publications are 154 predominantly in foreign languages. The monograph describes the risk factors, biochemical and biophysical diagnostic methods and treatment of preterm birth. Of the biochemical diagnostic methods, attention is paid to the Full-Term test - to determine fetal fibronectin, the Actim partus test - to detect phosphorylated insulin-like growth factor - 1, to the tests for placental microglobulin Alpha 1, progesterone and enzyme levels.
Combined diagnostic procedures, ultrasound measurement of the cervix/combination of three parameters, length and posterior angle of the cervix, diameter of the internal opening of the cervical canal are described.
The diagnostic methods for illustrated with the corresponding figures and echographic pictures.
If the biochemical tests are combined with the biophysical very high percentage /95%/ according to the author, it is possible to determine the risk of premature birth and undertake timely the appropriate tocolytic and other therapeutic measures, described in detail.
Dr. Kolev points out that the application of rapid immunochromatographic tests in 24-34 y.o. of the pregnancy additionally together with biometrics of the cervix makes it possible in 15 minutes to specify the risk pregnancy and to undertake the appropriate therapy. Of particular importance is the finding that an abortion is one of the most important factors for premature birth, and in our country the number of these abortions exceeds the number of births. The author compared his results with those of other international authors and they show a match of Full-Term test, Actim partus test and cervical biometry.
The monographic work of Dr. Nikolay Dimitrov Kolev, d.m., can serve both for the training of interns and obstetrician-gynecologists in their practice.
