The present work "Some Challenges to Pediatric Surgery" is dedicated to the diagnostic and therapeutic aspects of congenital anomalies in pediatric surgery, with a particular focus on malformations of the chest and bronchopulmonary system. It is a logical continuation of the previously published practical guide - "Congenital Surgical Diseases of the Neck in Childhood" and contributes to the systematization of knowledge in the field of a number of medical disciplines.
The common denominator of the presented nosological entities is the manifestations of respiratory symptoms, sometimes reaching severe respiratory failure. Usually, preoperative diagnostic planning does not fully correspond to the intraoperative finding. Intraoperative surprises are often encountered, which requires creativity and intraoperative improvisation. Due to the complexity of the anomalous variations, their difficult diagnosis, management and multidisciplinary requirements for them, we allowed ourselves to call the work „challenges“. The clinical and surgical characteristics of deformities such as: „pectus excavatus“, „pectus carinatum“, „pectus arcuatums“, Poland and Jeune syndromes, sternal defects, Cantrell-Heller-Ravitch pentalogy, as well as congenital rib malformations are described. Bronchopulmonary, other complex, and hybrid congenital anomalies with a high degree of diagnostic and surgical difficulty are presented, such as: congenital lobar emphysema, pulmonary sequestration, congenital adenomatoid malformation, horseshoe lung, congenital chylothorax, etc.
The need for an interdisciplinary approach is emphasized, including specialists in pediatric surgery, obstetrics and gynecology, neonatology, radiology, pulmonology, clinical genetics, otorhinolaryngology, orthopedics, vascular surgery, etc. Operative techniques are presented, some of which have been applied for the first time in clinical practice in Bulgaria. Two of the presented cases have been published in the Journal of surgery NY.
